Data Input Forms

Phase 1 of the MedMaps project involves conducting an inventory of all the maps held at the participating centers and compiling this information into a relational database. Below are copies of the data entry forms being used for the inventory in PDF format. The first form is for inputting information about the maps themselves, while the second is for materials (like atlases or scrapbooks) that contain one or more maps within them. Both forms show the fields that we are tracking for this inventory along with some data entry guidelines that are based on traditional library cataloging standards.
